This policy explains how information is handled when you visit the JAFS website or send an initial inquiry. It covers website inquiries, rather than the separate agreements and notices that may apply to coaching services.

Information you provide

Our inquiry form asks for your first and last name, email address, an optional phone number, and a message. We use information you submit to respond, discuss your questions or service needs, and communicate about possible next steps. Submit information about another person only when you have appropriate permission.

Keep initial inquiries limited

Please do not submit medical records, diagnoses, Social Security numbers, payment-card information, or sensitive details about children through the general inquiry form. If more detailed information is needed for services, we will discuss an appropriate way to provide it. The form is not monitored for emergencies.

Website hosting and the inquiry form

The website is hosted through ChatGPT Sites. The inquiry button opens a separate form hosted by Jotform. These providers may process technical information, such as IP addresses, browser information, and service logs, under their own policies. Jotform also processes the information entered into the inquiry form.

Cookies and analytics

JAFS has not added advertising trackers or analytics tools to this website. Hosting providers and third-party services may use cookies or related technologies for their own service operations. Review their policies and your browser settings for more information.

Information submitted through the form is available to JAFS through Jotform and may be processed by service providers involved in handling inquiries and communications. Information may also be disclosed when required by law or to address safety concerns. No website, electronic transmission, or storage system can guarantee absolute security.

To ask how an inquiry is stored, who can access it, or how long it is retained—or to request access, correction, or deletion—contact Kenya Gaines-Brown through our inquiry form. Identify your message as a privacy request and include only the contact details needed to respond. Some records may need to be retained for legal, contractual, or service-related reasons.

Children’s information

This website and its inquiry form are intended for adult caregivers and professionals. Children should not submit information through the form. If you believe a child has submitted personal information, contact JAFS so the matter can be reviewed.
